The compounded Buprenorphine ER medication requires a prescription to be called into Wedgewood pharmacy by Dr. Bradley. To aid in this process, please provide the following information to Dr. Bradley.
- Lab DEA License Holder name and address
- Number of vials (they are 5mL in size)
- Species
- Number of animals that will receive the medication
- Average weight of the animals
- Average age of the population
